Session Fees

  • Individual Therapy Session (50 minutes): $175

  • Couples Therapy Session (50 minutes): $200

  • Free 15-Minute Consultation: Available upon request

Cancellation & No-Show Policy

  • If you need to cancel or reschedule an appointment, please provide at least 24 hours’ notice.

  • Cancellations with less than 24 hours’ notice or missed appointments will be charged the full session fee ($175).

  • I understand that life happens-unexpected events, emergencies, and scheduling conflicts can arise. However, to ensure I can provide care to all my clients, I ask for at least 24 hours’ notice if you need to cancel or reschedule an appointment. This allows me the opportunity to offer that time slot to another client who may need it. Thank you for your understanding!

Payment Information

  • Payment is due at the end of each session

  • Accepted payment methods:

    • Credit Cards: Visa, Mastercard, American Express, Discover, JCB, and Diners Club

    • Health Savings Account (HSA) or Flexible Spending Account (FSA)

Superbill & Insurance Reimbursement

Cook Family Therapy, Inc operates as an out-of-network provider and does not bill insurance directly. However, I can provide a superbill upon request.

What is a Superbill?

A superbill is a detailed receipt that includes the necessary information for you to submit a claim to your insurance provider for possible reimbursement.

Questions to Ask Your Insurance Provider

If you plan to use out-of-network benefits, consider asking your insurance provider these questions:

  1. Do I have out-of-network mental health benefits?

  2. What percentage of the session fee will be reimbursed?

  3. Do I have a deductible that needs to be met first?

  4. Is pre-authorization required before reimbursement?

  5. How do I submit claims for reimbursement?

Good Faith Estimate

Under the No Surprises Act, you have the right to receive a Good Faith Estimate for the total expected cost of services. The estimate helps you understand what you may need to pay for therapy. If you would like a written Good Faith Estimate, please let me know.

Confidentiality & Ethical Considerations

  • Your privacy is important. All sessions are confidential, except in cases where disclosure is required by law (e.g., danger to self or others, abuse reporting).

  • I adhere to all ethical guidelines set by the Board of Behavioral Sciences (BBS) and HIPAA regulations to protect your information.

  • Payments are processed through Cook Family Therapy, Inc
